Over the Counter Medication for Students

 

1. Must be brought in by a parent.

2. Must have a student medication form, completed by the parent.

3. Must be unopened.  Do not send medication that has been opened.

Prescription Medication for Students

 

1. Must be brought in by a parent.

2. Must have a student medication form, completed by the doctor.
