Required Documentation

1. Copy of social security card.
2. Copy of CERTIFIED birth certificate.
3. Proof of residence.( power bill or lease agreement stating name and address)
4. An immunization form which can be obtained at the health department or your physician's office.
5. An enrollment card which can be obtained at the office.
6. Medical Form which can be obtained at the office.
7. Pupils on long term medication administered during the school hours may take medication only if a physician has stated in writing that the pupil should have a certain medication during the school hours. A form must be signed and on file before any long term medication may be given. Refer to the medication Policy in the Calhoun County Student Handbook and Code of Conduct.
